Our work involves addressing the patient’s social issues in cases where they are unable to do so on their own, where the family is unable or unwilling to provide this care, or where the patient has no family.
Who are these services intended for?
These services are intended for patients and their families in difficult social circumstances, particularly those with long-term health conditions or social disabilities, those who have been hospitalized for long periods or repeatedly, those with a significant decline in functional ability and self-sufficiency, people living alone, the homeless, and those subjected to mistreatment, neglect, or abuse, etc.
Scope of duties of a health and social worker:
- Information on social benefits. financial hardship, state social assistance benefits, benefits for people with disabilities, care allowances, etc.,
- Assistance in providing the follow-up health care. post-acute care hospitals, long-term care facilities, respite care and hospice care, etc.,
- Assistance in providing residential and outreach health or social services. home care services, home health care, mobile hospice care, personal assistance, nursing homes, specialized care facilities, shelters, night shelters, and housing for the homeless, etc.,
- Assistance in providing compensatory aids. adjustable bed, mattress, wheelchair with a shifted center of gravity, etc.,
- Assistance in asserting rights and legitimate interests. assistance in dealing with government agencies, arranging for pension payments to be forwarded to the hospital cashier’s office, etc.
