Naše oddělení poskytuje systematickou klinickofarmaceutickou péči na několika odděleních, pro všechna ostatní pracoviště nemocnice je zajištěna konziliární forma péče.
- Comprehensive evaluation and assessment of medication on admission to a healthcare facility, focusing on drug indications, dosages, dosage forms, routes of administration, drug interactions, and possible manifestations of side effects.
- Monitoring of pharmacotherapy in individual patients during hospitalization and at discharge.
- Individualization of pharmacotherapy with focus on pharmacokinetics/pharmacodynamics of drugs, function of elimination organs, patients' condition and presence of other diseases.
- Interpretation of drug levels (TDM) - particularly of antibiotics.
- Consiliary examination.
- Educational activities, cooperation in the development of recommended practices in individual departments
- Collaboration in the Palliative Team.
